I'm home now -- here's the exact quote from page 303 of Lactation:  
Physiology, Nutrition, and Breast-Feeding (sic).

"On the other hand, a false notion, current among breast-feeding enthusiasts, 
is "every woman can nurse."  While this philosophy is useful in combating the 
professional tendency to wean at the slightest obstacle, it appears likely 
that 1 to 5% of women experience lactation failure on a physiologic basis."  
(There is no reference for her statement). (Plenum Press, 1983)

Nancy is right -- this atatement has been quoted over and over again, even 
including the writer of the infamous July 22, 1994 New York Times article on 
"breastfeeding dehydration."

And, it was 15% of women in her lactation clinic in which she found IMS.
