If the baby is using a pacifier that is not being boiled daily (or
preferably just eliminated), this could be re-inoculating baby's mouth with
yeast spores only too happy to repopulate the area.
It was not specified that this is an exclusively breastfed baby either - in
any case, other objects regularly being placed in the baby's mouth are a
predisposing factor for thrush.
This baby is so young that it seems remote, but I'll mention it just in
case: if mother had expressed any milk and stored it when the thrush was
rampant, and then used this milk after treatment began, it can theoretically
reinoculate baby too. I have seen at least one case in which this was the
only conceiveable explanation for the recurrence of the symptoms of thrush.
