How about cat scratch fever?  Its often overlooked, but is a real entity.  My 
son got it shortly after getting scratched by a new kitty we adopted.  I had 
totally forgotten about it as a possibility when it happened.  My mother had a 
friend whose son had died many years before when I was a child.  Rocephin 
seems like a good thing.  Ask if her kitty is an outside cat.  If its a cat who 
is in all winter and suddenly rediscovers outside in the spring he might be 
clawing into bark or digging for personal reasons.
