I didn't see anyone respond to the question about a mom bf a 1 yr old and
adopting a premie. Depending on the size and gestation of the premie,
fortifiers may be felt to be necessary by the attending neonatologist. If
the baby is doing well perhaps he can take more volume and or more hindmilk.
Please refer to the recent discussions about protein needs for VLBW premies.
The mom's milk should not revert back to colostrum because no pregnancy has
occurred since the 1 yr old so the milk is really toddler milk, you could
say. I would suggest the baby be followed for good wt gains and any other
testing that needs to be done to make sure all growth parameters are met. Is
the baby getting to nurse directly on the breast?
