Hi Barb
Has J tried a nipple shield on the left side? I am thinking treat it you
would an inverted nipple. Turn out the shield half way, place over where the
nipple 'would be', then flatten out to get a good fit, (as per Barbara
Wilson's Clay's technique)
Did she try pumping at all on that side with her previous lactation? If so,
any relief of her engorgement? The fact she leaked would be encouraging I
would think.
Any sensation on or around the nipple? 
I would encourage her to try feeding on that side at least initially. You
will never know if you don't give it a go!!
Interesting case! Can't imagine a nipple actually falling off....ouch. Keep
us posted.
