Tina Smillie has some cool suggestions for this situation, she generally 
has mom put baby skin to skin vertically between her breasts, and let 
baby totally self attach.  He will bob his way down her chest to get to 
the breast and will latch when he's ready.  Try having mom feed, 
massage, cuddle and talk to him and then put him skin to skin for 
"dessert".  I've seen this work many times, if it does not work right 
away, baby often falls asleep on mom , and latches when he wakes up there.
