Hello,
I have been an on-again, off- again lactnetter for going on 10 years now.
I used to run a program for the Air Force, for new parents and now I am an
assistant manager of a maternity unit and the manager of their inpatient
lactation program.
I have just developed a charting form for my lactation nurses. It is
placed in the mom's chart and then one copy is placed in the lactation
nurses log book. We were going to also have a copy for the baby's chart
but were concerned with the amount of pressure required to write through
three copies (work place safety is a big issue for my company!). Anyway,
so far we love it. We have incorporated the referral form that the staff
nurses fill out, along with the actual charting for the lactation nurses.
It is placed in the progress notes section of the chart. We made it a
different color than the other progress notes so that it is easily spotted
by all caregivers.
