This looks like a terrible dermatitis. Does the rash follow the shape of
the breast pads? Whenever I see a sharply delineated rash in an unusual
shape, I'm suspicious about what item of that shape mom is using. Mom
might be allergic to something in the breast pads, the nystatin, or both.
Can she stop using anything on her breasts, sleep on a towel so she does
not need a bra or pads at night, and see if it heals?
Is there a dermatologist she could go to? This is certainly a case that
warrants in person medical attention.
