I like to prevent formula supplementation and perk the baby up by
starting mom hand expressing colostrum onto a teaspoon and feeding it to
baby within about 6 hours of birth, if baby has not latched. Esther
Grunis recommends that moms express 5 teaspoonsfull (about 5-7 drops on
a teaspoon) every 2 hours. Then follow the colostrum feeding with skin
to skin. Mom should start pumping if baby has not latched and her
colostrum is starting to increase in volume or by day 2. Research on
pumping for premies is clear that best milk supplies happen when mom
starts pumping within 6 hours of birth, and there is some evidence that
much milk supply calibration takes place on day 2.
Your idea to send this mom home with a pump (and instructions to use it
7 times a day and once at night) is a really good one if her babe does
not latch well before discharge.
