Hello!
I am working on a tough case- I'm not an IBCLC or a
LLL leader. I'm just working on my hours to become an
IBCLC- basically I need some advice from those more
experienced.
The case I'm working with is a mom and a nine month
old. Baby is healthy and has gained weight well during
his months of BF. This is mom's 4th child so she
expected no problems BF. When the baby was about one
month old, both of them were diagnosed with thrush.
Baby has been treated and is doing fine. Mom, on the
other hand, developed a lesion on her nipple that has
not healed up the entire time. She has extreme pain
when baby nurses and the lesion occasionally bleeds.
Mom has cut back nursing to 2-3 times a day and is now
supplementing baby (even the pump causes extreme
pain). Mom hopes the sore will heal so she can go
back to exclusive BF.
She has been in Dr.'s care: he has prescribed 2
different kinds of antibiotics which have not worked
(could this have further aggravated the yeast
infection?). She has been to her local LLL leader and
an LC who both recommended that she cut back feedings
on the traumatized breast and they have also had her
try Newman's Ointment- which also has not healed her.
She has tried various herbal treatments, none of which
have healed the sore.
The biggest complication is that the traumatized
nipple is continuously leaking. She'll leak through
double padding in the matter of hours. As a result,
she can't keep her breast in a dry environment
conducive with resolving yeast infections. I suspect
that any ointments she puts on her nipple might be
providing a better breeding ground for the yeast. She
has not tried gentian violet- I thought that might
help solve the trush, but as far as the continuous
leaking and the unresolved lesion, I am not sure what
to recommend. I am trying to emphasize her health at
this point as she is feeling very discouraged that she
has stopped BF the baby.
Can all of these complications be the result of yeast?
Any suggestions will be greatly appreciated.
