Jennifer,
My heart goes out to you with this issue. To me the magic words were
"swelling" and her inability to handle pressure above minimum. If this mother
is swollen enough, her nipple could also be swollen. When I have a
situation that involves a swollen nipple, I deal with the size of the nipple at that
time. The mother may normally be a 24 mm, but if swollen enough, she may
"temporarily" need a 27 mm. To me it makes all the difference making sure that
the mother fits her breastshields--at all times. I mainly work with working
women populations, and in my experience, inappropriate breastshield size (too
large or too little) can significantly reduce supply within one month. It
can also wreak havoc with mom's breast health in general.
