Interestingly, in the last couple of months I have had two cases of tongue
tie which could not be resolved with the simple frenotomy. Both cases were
similar to what Nicole describes. I went with the first mother and baby to
the ENT who does clipping for my clients and he said that there was nothing
to clip and that she could bring the baby back when he was older to have a
frenulectomy.
Mother had been pumping exclusively. She is still pumping but is putting
baby to breast more now, using a modified clutch hold which accommodates
his lack of tongue movement. She has a bountiful milk supply and baby
drinks adequately even though the latch is not proper. He is now 7 months
old and thriving. Mother has chosen to maintain an excessive supply so that
he can feed at the breast with ease. She makes high-fat bottles a couple
of times a day to off-set the amount of fore milk that he is getting some
of the time. Sometimes she pumps before feeding him but still has adequate
let-downs to satisfy him.
With the second one the ENT performed the surgery and baby is now feeding
very well. Mother was pumping and feeding baby with a bottle and after the
frenulectomy he went easily to the breast within a few days of having the
surgery. The parents are highly satisfied with it all as is baby.
Isn't it amazing that one can go for years not seeing a problem then there
are a cluster of them.
I have had other clients who have seriously damaged nipples and I recommend
pumping and giving a bottle until the nipples heal. And the babies do
return to the breast without much challenge.
