Heather, I use the same calculation as Laurie does starting on day 6-7,
depending on the baby and the baby's weight loss or gain. Earlier than that
I go by estimation and increase it gradually. With breastfeeding, babies get
something like a teaspoon per feeding the first day, increasing to about 1
ounce by day three, and more day 4 and on.
I almost never see a baby on the first week of life, I usually am only
dealing with train wrecks after day 7 where we have a baby that is 10-15%
under birth weight and so we have to push feedings for about 48 hours. What
I also tell parents is that the first day we start to feed more, baby might
not improve intake to the total amount, but the second day should be
increasing more, and by 48 hours after starting the intervention, I want to
see baby taking the correct amount. And we are talking about babies who have
lost so much weight that intervention is needed. Not breastfeeding babies
who are doing well. Many problems can not be solved until baby is eating
enough to gain strength and a desire to eat.
