If this baby hasn't improved enough, it needs to be seen by a doctor,
preferably a dermatologist. My son had a bad yeast diaper rash from
antibiotics as a baby with a large sore (maybe as big as the eraser on a
pencil) and the holistic medical doctor told me to use Waleeda baby creme
(with bee's wax and lanolin) to coat the sore so it heals, and over the
counter lotrimin on the rest of the diaper area. (This was 14 years ago so
the products may have changed.) It cleared up very quickly, like gone in 4
days. I would expect any treatment to work more quickly than what you are
seeing.
