Mom has 4 month old baby, exclusively breastfeeding, in 50th percentile.
However, mom has had recurrent mastitis and plugged ducts. This is the first time
she contacted me and do not know what meds were used for mastitis. At 3 days
her midwife clipped her baby's frenulum, mother had inverted nipples, used a
nipple shield for a while. Now, as of a few weeks ago baby has started to not
form a seal when latching. Also, milk pours out of the bottom of her mouth
while nursing at each feeding. (Mom does not feel like she has a strong let
down.) Baby will also not stick out her tongue anymore (mom realized that she
can't remember when she stopped sticking her tongue out and only thought of it
because I was asking what baby can do with her tongue.) Breastfeeding friendly
and supportive pediatrician thought baby was just choosing to not suck
because she eats too often---mom feeds whenever baby has interest. But with only a
50th percentile, doesn't seem like 'over feeding' to me. And, mom did try to
stretch the feedings but no change in the latch, seal, or dribbling milk.
Question: I will see this mom, what do I look for? What are some things
that don't show up until four months that can affect suck/seal and tongue
movement?
Who can I refer her to for a full evaluation? I am sure her pediatrician
will cooperate, just need to know what might be the best specialty to evaluate
this.
