This is what works well in moms I see with these kinds of problems:
Have mom pump both breasts as thouroughly as possible (preferably first thing
in the morning, that saves the rest of the day for practising), then have
baby feed at one or both breasts. baby will probably faal asleep very happy and
content on the fat milk. Then mom starts blockfeeding in a 3-4 hour pattern or
longer (but this mom apparently had bad experiences with that). Repeat the
pumping only if engorgement is huge again. In the first adjusting fase it can be
as often as every day, but that will subside to every other or every third day
and diminishing eventually.
What is important is this strategy is "no pumping" or "full pumping".
Sage can hasten the process of reduction in supply. I'm not used to using
tinctures, but use teas. I advise moms to make tea with 1 spoonfull of dried or
fresh leaves in 0,5 liter boiling water, let it there for 5 minutes (like in
making black tea) and drink 3 cups (teacups, not US measuring cus!) a day for a
couple of days. Mom must keep a close look at her supply and stop immediatly
once milksupply is getting down.
