One follow-up visit is included in my standard fee. That is, I do a
home-visit (usually 1.5 - 2 hours), several follow-up phone calls, and if I
or the mother feel a need, she can come to my home for a follow-up.

I like to keep it all included in one tidy sum - that way, the mother is
less willing to balk at another visit, and I am sure that I've done all I
can.
