Our hosp Admin now wants to develop Care Maps for all areas of practice.
NICU is the first section of Maternal-Child to be involved in this. Of
course, we want to include a Lactation element in all sections. Initial
discussions focused on 24-34 weekers, likely in 2 week increments. Our
NIDCAP specialist was directing the discussion as she has also been a NICU
RN. There was some discussion of "our population" which are infants born in
our hosp to parents who are immigrant, poor, little education, & 70%-90%
mono-lingual Spanish who can't read & need info repeated to them daily, or
more often. Contrast them to about 10% transfer-in babies with parents who
are well-educated, employed, homeowners, mono-lingual English who research
everything & expect to be at bedside with the medical staff at every stage.

I understand that Care Maps are lists of care guidelines and developmental
markers, and the care givers document everything directly on the Care Maps.
I believe the idea is that everything is taken care of in a timely manner
and the parents are educated and included at every stage. One problem that
is avoided is a deluge of last-minute teaching for the parents just before
D/C. I have also read that they can lead to earlier D/Cs which may or may
not be a good thing.

Does anyone have suggestions for me as I attempt to add lactation issues to
each stage of these Care Maps? Our big meeting will be all day the 20th &
21st. One day to hammer out the Care Maps and one day to work on staff
training issues.

Along with "our population", the staff likes the phrase "it's cultural" to
explain just about everything: "los dos", "no leche", "no sale", "no esta
lleno", "no quiso", etc.
