My main question is "what about foremilk inbalance, then?"
You may have heard that there is no such thing as foremilk/hindmilk
imbalance, but there is excellent evidence (see Hartman's research) that the emptier
the breast the higher the fat content of the milk. This is not in dispute.
However, Hartman's group also states that babies grow on volume and that we don't
need to worry about formilk or hindmilk when looking at growth. I personally
have never seen a baby gaining poorly due to foremilk/hindmilk imbalance. When
calories are too low resulting in poor growth, it is due to not enough volume
ingested.
Some of the behaviors we see in infants of mothers with oversupply may be due
to a relative lactose overload and I also think they may be due, also, to
these babies getting too much volume too fast. They grow well but they have a
tummy ache. Fixing the supply issues often fixes it. I also think that babies
that get too much milk too fast are prone to developing reflux.
