Dear all,
This will be lengthy. I have a mom who has been solely pumping for the past 3
months.
She wanted her baby to have the benifits of EBM. She does not wear underwire
bras; she
does wash her pump after every session & sterilizes all parts 1x a day. It
all started with a bout of mastitis on 6/14/04. Her ob put her on doxicillin x
10 days.
On 6/25 she c/o of burning nipples persisting. sl. soreness warm salt water
soaks were advised.
On 7/1 pt. called me again & I sent her back to her ob; I spoke to ob & asked
her if she could consider a culture to be done. Her md told her to apply
genitian violet.
7/5 md wanted mom to go on cipro for Serratia Marcescens from her culture.
7/7 mom said the baby's ped. said to pump & dump; mom not willing to dump
milk.
7/8 mom stated she's not uncomfortable at this time.
7/12 mom started feelinf the burning nipples with tingling again.
7/14 From here on mom seen by a md/ibclc in a nearby town.
Now mom had told her everything & she now has cracked nipples. md aware of
the bacteria mentioned in her milk. md felt it is thrush & put mom on nystatin x
3 weeks, along with diflucan.
8/4 I knew mom should be finished with meds. by now & didn't hear from her so
I called her.
mom has one more day left & states she still presents with some slight
tingling & burning
in breasts. Has no clogged ducts. Has been using a larger flange for the past
6 wks.
mom has no intention to stop pumping until the baby is at least a year old.
I have ran out of thoughts; I'm sure I've missed something. Does anyone have
any suggestions. I really want to help her.
Thank You in advance,
Elaine Miranda RN, CBC, CCE
