Sorry for the delay in posting.  Hopefully, by this time, you have it all
figured out and this mom is feeling better.  Having heard of a similar case,
the cause was determined to be what I call "foldover."  Mom had elongated
nipples and would unknowingly fold them over when putting her bra on.  The
deep fissure occurred once baby was nursing and the creased area began to
stretch and weaken.  This continued on for about 9 months or so until a LLL
Leader figured it out.  Once mom gave special attention to how she put her
breast away after each feeding and there was no more "foldover", the
fissuring stopped!
Isn't it funny how sometimes the solution is so simple?
