Ann, are we sure that mom had a truly good supply in that right side
from the start? Or if she did, that it was draining properly from the
start?
I had one mom for whom (with two babies) the milk came in normally on
both sides, but neither baby nor pump could get much out of one side.
She involuted over a few weeks' time, and given that she had an unusual
nipple, I assumed that the ducts just did not connect to and/or drain
well through that nipple, resulting in no milk removal and involution.
The same thing can happen when there has been breast surgery; the milk
can come in, but if it is obstructed and can't be removed readily, the
breast will shut down and involute.
You might take a detailed history from the mom as to how that breast has
seemed to function and how much swallowing baby has done at that side
earlier, as well as how long baby has been willing to nurse at that side
until now as compared to her good left side.
