my photo release is very simple, it says:

The undersigned has hereby granted permission to roni chastain to take and
use photographs of myself and my child. This permission has been granted without
limitation or condition.

Date;
i also include the babys birth date, so i remember.
If anyone has others, i would be interested in seeing them. I have had photos
published with this permission slip.
