Dear Friends:
    In the hospital today, I met a woman who had just had her 2nd baby. She
nursed her first baby x 13 months. She described severe first minute latch-on
pain, for 2 months. She went to support groups (in California) and got as much
help as she could find. Her nipple was never mashed or misshapen or burning.
No signs of vasospasm or thrush. Just 20-30 seconds of excruciating pain when
baby first latched on. Expressing milk before baby latched didn't help much. It
disappeared at 2 months, when she was ready to quit. Meanwhile, baby thrived.
Once the initial pain was gone, the rest of the feed was fine.
    She is upset to encounter this very same symptom with this new baby, even
at the first nursing.
    I suggested warmth to the nipple before nursing, but couldn't think of
anything else. She is committed to breastfeeding and is just hoping that she
won't have 2 months of first minute misery again.
    Any ideas?
