On the question of domperidone's working only for mothers with low
prolactin levels:

I have had several clients who used domperidone for lowered milk
supplies--following adequate supplies-- several months after birth.
A  couple of them had  infants more than a year old. Their milk supplies
rebounded within 24-48 hours. Others have used it following an illness in
which their supplies dwindled.

Of course, these mothers may have lowered prolactactin levels at that point
creating the reduction.  What do you think?

Also, I had a client whose baby with Down Syndrome never succeeded in an
effective transfer of milk. Mother pumped every 2 hours for 9 months.  She
had experienced low supplies with her older children and relied on
domperidone to keep her supply adequate for this little one.

She found that the 20mg/6 hour interval was not adequate to sustain her
supply so she took 20mg every 4 hours instead. While pumping every 2 hours
she found that the pumping an hour *after* taking the domperidone provided
more milk (up to twice) than the pumping right before the next dose of
domperidone was taken. So she timed her pumping accordingly. I found that
interesting.
