Read the baby (cues), not the protocol, book, etc. There is variation among all the special populations cited in the post. They come with various physical issues and abilities to BF. If there is a mechanical problem that can be fixed, fix it; if not currently fixable, protect production so baby gets mom's own milk. If CNS maturation is the issue or infection, etc., protect production while waiting for baby to mature or heal. All along be sensitive to baby's cues. Baby always is the best guide...
