Valerie, we don't use consent forms, but moms sign a pretty broad one when they apply for WIC. We never used consent forms in the hospital I worked for either. I don't use a consent form when I do PPLC -- BUT I do talk it over with mom before using it. The pros and cons, why I am wanting to try it, and I always give mom the LLLI handout, show them the Medela product handout, and go over it with her when we are done, and do careful follow up. And I always go over weaning from it and encourage them to call me back if they have trouble weaning from the shield later on. I explain to mom her options in using it or not using it.

Oh, and follow up is mandatory for WIC staff when giving out a nipple shield! I think follow up is what is often left out when hospital nursing staff gives mom a nipple shield.
