Are there formulas that can only be obtained through prescription by a PED? Or prescribed by purchasing components and mixing by parents?

Hello All!
I am updating a project that I completed for a BreastEd module (no connection other than as a student) with local formula prices ... but so far I haven't been able to get any doctors locally to answer the question "which is the most expensive/specialty formula you recommend/'prescribe'?" Does anybody out there know?
The project, which I would like to begin using as a handout to mothers in educational sessions, is a chart showing the costs of breast vs. formula feeding in the baby's first year.
