could this be a galactocele (milk filled cyst)? i do not have my reference
texts nearby, so can't look up the word, but the description sure sounds
similar!

the falling asleep thing sounds odd, but 45 minutes every 2-3 hours sounds
pretty normal. has baby been gaining slowly, or is baby just small to
begin with? Did you watch her nurse?
