Regarding Lynn's case, I have observed that mother's with over active
let-downs and abundant milk supplies the babies generally do better at night.  The
differences usually are that they are side-lying nursing which allows the milk
to pool in baby's check or dribble out of the mouth before the bolus reaches
the back of their throat.
During the day mothers tend to use an upright position and also tend to be
pushing the baby closer to them (this is not in a forceful way but there is less
tendency when side-lying).  The football hold or clutch hold is the worst for
these little ones because the milk is delivered too forcefully to the back of
the throat before the baby can compensate.
Babies have memory of negative experiences so even with mom taking some of
the milk off or making other changes to help during the day, a baby may take
awhile to realize that things are better.
I like to encourage the rebirthing bath for these couplets until things are
better.
