I agree with Pat Gima that vasospasm is the probable culprit here.
A very extreme asymmetrical latch is usually very helpful. When the mom
gets the latch "just right" there is no pain. Sometimes bf is so
painful that these moms need to be given "permission" to feed expressed
milk for one or more feedings a day until they and the baby become more
practiced at latching just right.
Warming the nipple after the baby releases is also good. A Lactnetter
(sorry, I forget who) suggested a rice sock heated in the microwave.
This has been a great suggestion for my clients. The warm palm of the
mom's hand can work too.
