We also had to address this when we wrote a supplementing P&P.  We
have one for medically indicated and one for mother's choice.  We
had to eliminate the reference to being sure she was making a "fully
informed" choice because it was decided that "informed choice" was
exclusively in the realm of the docs (I won't get int that further
here).  However, the gist is that when a mom requests baby be
supplemented in the absence of a medical indication she is informed
of the possible drawbacks.  These include her body now knowing baby
"went to the competition" and being behind in supply the next day,
potential for nipple confusion (preference or whatever you want to
call it), delay in the "milk coming in" as well as all the fact that
there are phisiological consequences to "just one bottle".  Then we
talk about giving bottle in such a way as to reduce some of the
differences if she still wants one given.
I think your approach is reasonable.  The basic idea is find out why
she wants to supplement and deal with that problem first, be sure
she knows there are potential drawbacks, then do whatever you can to
reduce the negative efects.  The trick will be to get the nurses to
follow the Policy!  Some seem to thing that "Are you sure you want
to do this?" is all that's needed to constitute informed choice (or
whatever you want to call it).
