Sorry for the partial message prior to this.

I have been contacted by a mom who has been breastfeeding for over 4 years
and just began tandum nursing about 2.5 weeks ago with her newborn.

She describes a swollen white area about the size of her pinky fingernail on
her areola right at the nipple. When she pushes on it she says it feels like a
vein as it seems to bend underneath. It doesn't cause pain when at rest but
is very painful with breastfeeding ("like a raw nerve")

She has had plugs before and doesn't think it's a plug or milk blister. Her
midwife thought it might be a varicose vein and the doctor thought it was
herniated breast tissue

Does anyone have experience with this or know of any treatment. All I could
find were references to surgery to repair it if it is a herniation. I suggested
she get it checked by a plastic surgeon to see if it is a herniation (since
the references I found were in plastic surgery journals) and discuss options
for repair or relief of pain until she is ready for repair.

I'd really appreciate any other thoughts or experiences.
