I *really* look forward to reading this information.  As a non-hospital
based LC, my impression from our local hospitals and the mothers and babies
I see is that there are discharge guidelines but they involve supplementing
baby with abm if baby does not latch a certain number of times in a certain
number of hours.  Mothers and babies are frequently discharged without baby
having ever latched and I have yet to hear about a discharge plan that
included a referral to a local LC.
