Sounds like mom's supply or baby's milk transfer is now too low if baby
is not gaining weight. When a baby can't handle the milk flow, it is
not always mom's fault. Babies with tongue ties (even mild ones) are
likely to have difficulty with a NORMAL milk flow, due to difficulty
shaping the tongue to hold a bolus of milk for a controlled swallow.
Reducing mom's milk supply in these cases can cause failure to thrive.

The first thing I would do is help this mom make feedings as efficient
as possible for the baby, and make sure baby is getting enough milk.
Mom might need to increase her supply by pumping or feeding more often.
Baby can be helped to handle the flow by positioning him prone on mom's
chest while she semi-reclines, or feeding him sidelying so he can
release excess milk into his cheek pouch temporarily. Some infants feed
better while being walked in a sling type baby carrier.

The baby's tongue function, oral anatomy, and oral motor skills should
be checked by someone who is expert in this area.

While it is always possible that baby is sensitive to something in mom's
diet, that is not the most likely cause of baby's distress.
