Jennifer, one suggestion:
Because these twins are discordant and of quite different sizes, I'd suggest "assigning" each baby to their "own" breast--:
Right now, while one twin is a more vigorous feeder, and we are still in colostrum phase, it is fine to be alternating twins at alternate breasts, to be sure that more vigorous twin helps get things started well on both sides.
However, very soon, because they are of different sizes, they will have different appetites and needs. If you keep alternating breasts, the little twin may end up doing a lot of foremilk feeding, because the bigger baby may push up the general rate of production too high. Alternatively, the smaller baby may
have a more rapid growth rate and actually need to feed more than the bigger baby. When each has their own breast, each baby is able to regulate production precisely to their own appetite and thirst.  Neither you nor the mother needs to micromanage and think about skim milk and cream, volumes, etc., you just
give each baby a breast, and, once feeding is established, they take care of the rest.  This is not to say mom has to be incredibly rigid about this, each can nurse on the other's side now and then, when the situation of the moment calls for it (eg, one breast uncomfortable, that baby sound asleep, etc.), as
long as in general they are each thought to have their own sides. They can be as flexible as a family with regular places at the dinner table, who nevertheless shift now and then for this or that reason.
