What a great history, Rachel.
Now that you have given us such a complete picture, I can say that it
certainly sounds like a sensory-based feeding aversion.  Perhaps from
pain during early feedings, perhaps due to severe reflux, perhaps due to
difficulty handling the flow of milk (a swallowing issue perhaps).  An
occupational therapist would be the person to deal with this in the US.
  Do you have OT's in Norway?
Marjorie Meyer Palmer (author of the NOMAS - Neonatal Oral Motor
Assessment Scale) spoke about sensory based feeding issues at a
conference called The Child with Special Needs (it's an annual
travelling conference, like ILCA's) when it was in Washington DC a few
years ago.  It was a preconference day devoted to feeding issues, and
she was the sole speaker.  I know cassette tapes were made, but I don't
know who sells them.  Perhaps that is enough info for you to track them
down.
