We have had a similar concern.  A sister hospital has a very
knowledgable person who has been doing mich of their breastfeeding
education and help for many years.  This person doesn't have the
educational credits necessary for IBCLC and isn't really interested
at her age to pursue it.  We have been struggling with just what her
scope of practice should be.
The other issue is that at our hospital, one of the IBCLCs does not
have an RN (or other health care liscensure) and another is a nurse
certified in another country, but not in the US.  The problem is
with "receiving orders from an MD".  According to hosptial policy
(and supposedly state statutes) only LISCENSED health care personnel
can do this.  (FYI-OTs, PTs etc. are liscensed.)  So when a doc
phones in an order for a consult, these 2 IBCLCs must get an RN to
officially take the order.  Usually it's not a problem except on
weekends when they have to get someone from the OB floor to do so.
We're trying to get around this, but no luck so far.
