Sounds like difficulty handling the milk flow...
What I find works is to give baby total control when at the breast.  Mom
can hold baby's body and offer breast, but should not touch baby's head,
and should let baby off breast immediately if baby wants.  Try
sidelying, walking while feeding (sling is helpful here) or reclining,
to ease milk flow a bit.  If baby wants to suck for 20 seconds, fine.
Mom just gently offers frequently.
