We do not have mothers sign a consent form when using nipple shields in the
NICU. They are used under the direction of an LC and by the time the baby is
discharged we have been able to fully evaluate the baby's ability to
breastfeed effectively with a shield as well the mother's ability to use it
correctly and to assess her baby' s intake.  Many of our babies transition
from the shield to the breast before discharge, however.

And in answer to another post, only LC's evaluate the need for and give
nipple shields to our post partum mothers. We generally do not give them in
the hospital, preferring to wait until the milk is in, since we can then
evaluate much better whether they are working for a particular dyad. If they
are started in the hospital a follow up outpatient appointment is made in
several days to re-evaluate and we have mom's pumping as well.
