Some suggestions:
If it is overactive letdown then often the baby will tolerate the expressed
milk from a bottle (or other alternate route). Or have the mother initiate
the letdown and put baby to breast after that initial big spray subsides, if
that is the problem. If the symptoms go away on nutramigen and resume upon
bf again, then it is probably something the baby is 'reacting to' in the
milk, i.e. a protein. The mother may need to be more precise about going off
dairy and also look at what else it might be. Check the archives about the
diet mom should go on (something like turkey, squash, rice, lamb) and stay
there for a while then start adding foods back. It could be any no. of
things baby is sensitive too.
