Actually, Sally, my experience has been that the younger docs are having absolutely no problem in cutting frenulums that are suspiciously causing problems with breastfeeding. All of the ones here seem to have been taught to do them somewhere down the line - don't know if it was in med school or in the residency part of their training, but I am very happy about it!

I have one case of a baby with moderate tongue tie, and baby is only able to nurse with a shield. I assessed the frenulum in the hospital, and although it was moderate in severity, was certain it was causing the baby's tongue to snap back. No central grooving of the tongue was visible, and there was a slight notch in the tongue tip. Referred to baby's dr (an older guy) who said it was not a problem. Baby is now three weeks old. I have referred the mom to an IBCLC pediatrician in town, and she will be seen in three days. Can't wait to see the outcome on this one! (and you can bet I will report the outcome to the other pediatrician!)
