Regarding the premie with tongue-tie:

See if a local dentist will evaluate and clip the frenulum. Dentists
(especially pediatric dentists) are qualified and licensed to evaluate mouth
anatomy and perform minor surgery as needed.
Remind mom and the baby's pediatrician doctor that a short frenulum that
inhibits the tongue's ability to move normally, making bottle feeding even
MORE dangerous than breastfeeding because of the higher risk of aspiration.
The baby with a short and/or tight frenulum can't STOP milk from flowing in
order to breathe. This puts the baby's nutrition and respiration at risk.
Meanwhile, keep her pumping rather aggressively. This is a feeding emergency
for the baby until someone takes this issue seriously and releases the
frenulum. Check Brian Palmer's web site
