I would use cabbage leaves applied constantly, changing when they wilt.
Also ibuprofen--200mg every 4 hours.
I had a similar case (both breasts) and the above reduced the edema
adequately for pumping to begin. We went through 3 heads of cabbage the
first day, changing frequently. We could smell the cabbage cooking. After a
couple of hours there was a beginning of a flow, then more each hour. Mom
continued with the above for 3 days with increasing comfort each day. She
went on to feed well on both breasts, with baby thriving.
