Hi Jessica,
I think this is a case where, as a last resort before quitting, I would suggest a nipple shield. There are those cases where when all else fails, for some reason or another, it works. It allows us to see the latch a bit better, and also milk transfer.  It may give this mom some rest after I imagine she must be experiencing pain although you didn't mention any pain on the nipple area, just mastitis (which in itself we know can be painful.) Might relax her when she can see actual milk.  Does he have a receding chin? any abnormality in oral cavity.?
