#1- Mom of 4 week old has pain, usually at the end of the day in breasts and
nipples, thought to be yeast. Mom used diflucan, gentian violet, nystatin,
APNO. Mom also pumped for 3 days. Pain diminished for a day or two but
returned. I watched baby nurse and examined nipples. Baby looks good except
when I did an oral assessment, he gagged almost immediatly. Although the mom
describes extreme pain in nipple, nipples do not look bad at all. On a scale
of 1-10, 10 being the worse, I would say 2, maybe? Mom keeps suggesting that
the nipples are really bad "look, arn't they horrible?",bit I don't see it
as so horrible. She has latched baby on with me and said it didn't hurt. Uses
both breasts, clutch and cradle. Clutch is better for one side, cradle the
other.
Her breasts are extremely firm, Bcup, small, little short nipples. Baby
latches over entire areola. My suggestion to her was that her nipples are not
very 'elastic' and the baby isn't able to pull the nipple back far enough due
to this. This may be why she describes the nursing as a 'gnawing' not a
suckle. Therefore, the pain. Any suggestions for this mom? She is ready to
quit. I told her I would ask around for more suggestions.
