I think that happens very rarely - if anything is left blank, the UCC
hounds the nurse until it's complete. I check our forms we use with the
moms against the list to determine what percentage we see, and we rarely
have one that's not on the list. When we do, she is added to the
stats. Next time it happens, though, I'll check the birth log to see if
that's why. We do have moms that say they plan to do one or the other
and change their mind even before the first feeding, or the ones that
plan to breastfeed once home and bottle feed "until the milk comes in",
so when the nurse asks, they say "bottle".
