Hi all, I am interested in knowing if any lactnetters in practice have put
together assessment and documentation forms for use in their practice
(hospital or private)? I am a private practitioner and do mostly home visits
and am finding that the "standard" forms available are very long and add tooo
much time to a visit. I have created some of my own to fit my needs, but now
that I am focusing on insurance reimbursement, I find I need more detail to
support the needs of those darn insurance companies!! For anyone that is
getting insurance reimbursement, would you be willing to share your
documentation forms? ( I see why taking a class in medical coding might be
helpful. they read the chart, pull information to create the insurance bill
(ICD9/CPT codes). If the chart supports the codes, great.. if the chart
doesnt support a code then you get less reimbursement or nothing!! That
makes a difference for moms when they are submitting those superbills.

As a side note, I continue to learn soo much from this listserv and am
greatful that it has grown as it has. It remains professional and a great
resource of information. So, can those in the know, start a discussion on
how documentation supports insurance billing! The more I do the more I
realize how much I don't know about the relationship of documentation to
insurance processing.
