Several people e-mailed me requesting I send out a
copy of my facility's documentations sheet for infants
who have had vacuum-assisted delivery devices used on
them.  I thought I had saved the e-mail addresses, but
unfortunately, I can only find one at the present
time.  If you are interested, please e-mail me again
include a mailing address as I am relatively
techno-clueless & am not sure I'll be able to
successfully send a computer copy out.  :)
