Since colostrum is lower in fat than mature milk, perhaps she can
breastfeed initially, then start replacing some fat from expressed milk
with medium chain triglyceride oil once her milk becomes abundant.
Perhaps she can still feed at the breast at the beginning of feeds when
the fat content is lower, then give the skimmed pumped milk with MCT oil
another way, then pump the higher fat milk and skim it for the next
feeding.
